(B) Select the month for which you are claiming overages
(C) Complete your name (last name first) and Employee ID Number
(D) Fill in the total number of students within the highlighted cells
1. Please note that students teacher assistants (STAs) are not allowable when calculating overages
2. In the event you get the greater of the total overages by period or on a daily basis, the worksheet will apply that greater amount. Applicable only to middle and high schools
(E) For teachers teaching on a period basis, please input the periods you are instructing students
1. Not applicable for teachers teaching in self-contained classes (i.e. grades K-6)
2. Please note that the periods need to only be input once, a formula is in place to populate the rest
(F) Print, sign, and submit your class size overage claim sheet, along with copies of the Weekly Attendance Reports (WARs), on a monthly basis to your site administrator/supervisor for approval per the time line memorandum dated August 28, 2012.
